What to bring
- Photo ID and your CDL, or your permit if you do not have the license yet.
- A list of every medication you take, with doses and the prescribing doctor.
- Glasses, contacts or hearing aids if you use them. Bring them, do not just mention them.
- If you have sleep apnea, your CPAP compliance report. If you are diabetic, your recent A1C and a blood-sugar log.
- If you have a heart condition, a stent or a pacemaker, bring the clearance letter from your cardiologist. Without it you will be sent home to get one.
